Assessment

Approval & Waiting Period

Once approved, you will receive a notice of decision confirming your eligibility and indicating your priority level. You may need to wait until a package becomes available based on your priority and availability.

Finding a Provider

When a package is assigned to you, you have 56 days to choose a home care provider. BASSCARE Home Care is here to support you with a range of services tailored to your needs.

Create a Care Plan

Together with BASSCARE, you will develop a personalised care plan outlining the services and support you require.

Service Delivery

Once your care plan is in place, BASSCARE will coordinate and deliver the agreed services, regularly reviewing and adjusting your care as needed.

Support at Home groups funded ongoing aged care services into three main categories, each with defined service types.  Each category is tied to distinct funding arrangements and co-contribution rates, with clinical supports generally fully funded and independence/everyday living services subject to client co-payments according to assessed ability.
